You may use this form if you have trouble accessing or using online materials, including:
• Images or graphics that don’t include descriptive alternative text (alt text)
• Videos that don't contain captions or transcripts
• Documents or PDFs that aren't compatible with screen readers
• Text that’s difficult to read due to low color contrast
• Links, buttons, or menus that can’t be used with a keyboard
• Charts, tables, or forms that aren’t properly or clearly labeled for assistive technologies
• Emails with inaccessible attachments or images without descriptions
